Continuing Education Accreditation
The American Society of Health-System Pharmacists is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. This activity provides 1.5 hours (0.15 CEUs) of continuing education credit. Program number 204-000-08-448-H01P.
This activity is provided free of charge. After participating in the activity, pharmacists may complete the CE test online at the ASHP Learning Center. A passing grade of 70% is required to receive continuing education credit for the activity, and the CE statement can be printed immediately.

The program is beneficial for those who treat, instruct, or otherwise participate in the care of ambulatory and hospitalized patients with or at risk for venous thromboembolism.

Learning Objectives
After participating in this knowledge-based educational activity, participants should be able to
- Describe the clinical and economic burden, pathophysiology, clinical presentation, and diagnosis of VTE, and list common risk factors for VTE.
- Compare and contrast the pharmacology, monitoring requirements, and role of various anticoagulant agents in patients with or at risk for VTE.
- Assess the risk for VTE in both a hospitalized patient with acute medical illness and a surgical patient and recommend appropriate prophylactic anticoagulant therapy.
- Recommend appropriate anticoagulant therapy and monitoring parameters for a patient with VTE.
- Explain the rationale for ACCP guidelines for anticoagulant therapy in patients with or at risk for VTE based on the results of key clinical trials.
- Identify at least one performance measure that can be applied in the practice setting to improve care for patients at risk for VTE.
